The Affordable Care Act authorizes an Annual Wellness Visit (AWV) for Medicare participants.A required component of the AWV is a health risk assessment (HRA). The assessment can be completed in less than 20 minutes online, telephonically, or in paper/pencil format. Upon completion of the HRA, a personal report is generated that provides a detailed health summary for participants, and a physician summary report for medical follow-up. Wellsource also offers a collection of web-based self-management tools to further educate participants on their health interests and assist them in making lifestyle changes if necessary.
