The assessment can be completed in less than 20 minutes online, telephonically, or in paper/pencil format. Upon completion of the HRA, a personal report is generated that provides a detailed health summary for participants, and a physician summary report for medical follow-up. Wellsource also offers a collection of web-based self-management tools to further educate participants on their health interests and assist them in making lifestyle changes if necessary. The new Medicare question set, personal report, and health information resources satisfy the requirements for Health Improvement Product (HIP) certification by the National Committee for Quality Assurance (NCQA).
